Overview

Admission portals open on short notice and close within days, so having your documents scanned and ready before applications begin can save your seat. The list below covers what Pakistani medical colleges usually ask for. Your prospectus is the final authority, because requirements differ by university and category.

Core academic documents

  • MDCAT 2026 score card or result certificate.
  • Matric (SSC) certificate and detailed marks sheet.
  • FSc (HSSC) Pre-Medical certificate and detailed marks sheet for both parts, or provisional result if applicable.
  • Equivalence certificate from IBCC for candidates with O/A Levels or foreign qualifications.

Identity and eligibility documents

  • CNIC of the candidate or B-form if no CNIC yet.
  • CNIC copies of father or guardian.
  • Domicile certificate for the province or Islamabad you claim seats from.
  • Recent passport-size photographs with the specified background.
  • Nationality or citizenship proof where the prospectus requires it, for example for overseas Pakistanis.

Category-specific documents

These apply only if you are claiming a special quota.

  • Hafiz-e-Quran certificate from the recognised authority, where bonus marks apply.
  • Disability certificate for reserved seats.
  • Armed forces or martyr quota certificates.
  • Sports or other special category proof, if the prospectus offers it.
  • Reciprocal or overseas seat documents, such as passport and visa or residence evidence.

Payments and forms

  • Application processing fee challan and its paid copy.
  • Admission fee payment proof after selection.
  • Affidavits and undertakings. These are typically on stamp paper in the format given by the university.
  • Medical fitness certificate, where required at joining.

How to prepare in five steps

  1. Scan every document in colour at a readable resolution and name files clearly, for example matric-marksheet.pdf.
  2. Check the file size and format limits on the portal and resize in advance.
  3. Get required attestations early from an authority accepted by the university. Follow the prospectus rather than assuming a bank can attest academic records.
  4. Verify that the name, father's name and date of birth match across all documents. A mismatch is a common reason for rejection.
  5. Keep one complete set of originals and the required number of photocopy sets for report day.

Mistakes that cause rejection

  • Blurred or cropped scans.
  • Domicile from the wrong province or an expired one.
  • Unattested photocopies where attestation is demanded.
  • Missing equivalence certificates for O/A Level students.
  • Uploading the wrong fee challan.

On report day

Bring originals plus photocopies of everything you uploaded, passport photographs, the fee challan, and your selection letter. Colleges verify originals before confirming your seat, and a missing original can delay or cancel admission.
